是的，句（2）确是一强调结构，应用 that，因为它所强调的是状语 in 1949。这里如没有介词 in，只有 1949，其后即须用 when，从而就不成为强调结构了。照此而论，上述句（1）自然应是一强调结构，而 when 也自然应改为 that 了。但是，句（1）却有点特殊。在英语里，after two o'clock和two o'clock 一样（却和 in 1949 不一样），也可以用来表示一个时间单位，例如：
(3)— What time is it?
— After two o'clock.
故此，上述句（1）并没有错。它不是一种强调结构，其意为：当外科医生到达 Glens Falls 的医院时，已过凌晨两点了。
当然，在一定的上下文中，上述句（1）也可能是一强调结构。那样，when 就用错了，应改为 that，而在强调结构中用 when （以及 where），则是英美人常犯的错误。
